Tapeworm is a Side Job in Cyberpunk 2077. Tapeworm can be acquired from Johnny Silverhand. This side job activates at various points during Act 2 and will disappear from the journal until it is reactivated. Phases one to four become available after completing Automatic Love, Life During Wartime, Search and Destroy, and Transmission. Depending on the order in which you complete these quests, the dialogues will occur in different locations, except for the final one. Completing Tapeworm will trigger the beginning of Act 3. Completing Side Jobs (Side Quests) in Cyberpunk 2077 rewards you with experience points and may reward you with items. Side Jobs can also influence the main story outcome.
Looks like "know thyself" has become a two-for-one deal. If it's true we're becoming one person, I suggest we keep an open dialogue. And by the way, I'd be happy to explain why the only shot you got to survive is to reach Mikoshi and stay in one piece along the way. You'd be doing me a huge favor. After all, if you're not good at following my instructions, I have to inherit whatever's left of you.

- After "Transmission": On your way out of the chapel after speaking with Alt.
- After "Life During Wartime": Following your conversation with Anders Hellman.
- After "Search and Destroy": After talking to Hanako Arasaka.
Hotel Pacifica
- At some point during the questline, V will experience a blackout caused by the malfunctioning relic, and you'll wake up near a hotel in Pacifica.
- Follow Johnny: Upon arriving at the hotel, simply follow Johnny to a hidden room where you'll locate a cache of Johnny's old belongings.
- Conversation with Johnny: Before the conversation ends, Johnny will ask V to help him take revenge on Adam Smasher. You'll be given a choice:
- Allow Johnny to take control of V’s body: This will lead to Johnny's plan to meet Rogue and exact his revenge.
- Deny Johnny control: You can refuse Johnny's request to take over your body, but it won't prevent you from completing the quest.
You'll finish Tapeworm after your conversation with Johnny and leaving the hotel. Depending on your progress in the story, you may now enter Act 3 and continue with the main job, Nocturne Op55N1.

Spoilers ahead: Does this mission affect the ending? Particularly whether or not V and Johnny's plan involves giving the body to V or Johnny?
"But from the ashes sprouted a powerful friendship. And in time, V found that they could almost tolerate them, much like a fat man tolerates the tapeworms in his intestines. And like those tapeworms, that engram burrowed deep into his innards and gorged themselves on his leftovers, until they began causing abdominal pain and diarrhoea. I realize the metaphor's breaking down a bit here, but Granny Dapper didn't raise no quitters."
