Worked for me for a while...that is until I got a pair of Mantis Blades. It seems that this mod, at any rarity, and the Blood Pump, also any rarity, will not work once you get an arm cyberware (i.e. Gorilla Arms, Mantis Blades, Monowire, Projectile System). If I had known this, I would have simply ignored getting Mantis Blades or any arm enhancement, as these are more powerful in my opinion (I'm running a blades/cold blood build and Kitanas are better than Mantis Blades in nearly every way).

It's broken, only worked the first time, then never activates again. Complete waste of a slot until they fix it.

If you are playing on very hard and take more damage than your max health in a single hit, this thing won't activate. Just take note of that if you're wondering why you got oneshot and didn't get healed. Figured this out because the slowtime if health below 25% cyberware follows the same rule.
